THIS POSITION IS LOCATED IN A TOBACCO-FREE ENVIRONMENT.Major Duties:DUTIES: The purpose of this position is to function as a Diabetes Nurse Specialist. The program provides comprehensive diabetes education services. The incumbent is a member of a diabetes team. The primary purpose of this position is to provide diabetes education and counseling to people with or at risk for Type 2 Diabetes Mellitus in the Four Corners Regional Health Center service area as part of a comprehensive diabetes program. The incumbent provides patient care services to a mix of young adults, middle-aged, older adults and geriatric patients and occasionally sees obstetric and adolescent patients. Provides outpatient education and counseling to individuals and families according to the curriculum, policies and procedures per Diabetes Education & Counseling Center (DECC). The incumbent assesses individual learning needs inclusive of psycho/social/cultural aspects and implements an educational plan to meet those needs. Teaches the pathophysiology of diabetes, diet, exercise, medication, diabetes mellitus/gestational diabetes care for management of diabetes. Will serves as the diabetes class coordinator and primary instructor for group classes. Responsible for: diabetes class curriculum development; diabetes patient educational needs assessment and goal setting; development of class objectives, content, method and evaluation; maintains files for the diabetes classes; organizes a 6 month follow-up session with patients completing classes to evaluate patient goals and outcome of self management; maintains patient evaluation of classes. Will perform clinical procedures as needed such as administering medication(s), taking vital signs, measuring height, weight and testing capillary blood glucose. Coordinates diabetes care and patient visits with other departments for prevention of diabetes-related complications. Plans and participates with other professionals and agencies involved in providing services to people with or at risk of diabetes, families and the community and where appropriate, coordinates services provided. Maintains registry for case management. Participates in review, selection, development and evaluation of appropriate diabetes education materials that are at the patient's age level, literacy level and are culturally relevant. Establishes goals, objectives, standards of care, and evaluation technique with other team members. Contributes to the overall (DECC) planning, policy and procedure development, program evaluation and performance improvement (PI). Provides community diabetes education to groups in various community settings. Participates in the assessment of community needs for educational services and the design and implementation of outreach educational programs to meet those needs. Provides consultation on diabetes to other health care professionals. Maintain documentation of education and counseling services according to facility and Center policy. Provides patient care according to the Standards of Care for Patients with Type 2 Diabetes in IHS. Establishes and maintains liaison with national and local diabetes associations. Participates in community events as schedule allows. Performs other related duties as assigned.Qualifications:

BASIC REQUIREMENTS: Education – Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant.
IN ADDITION TO THE BASIC REQUIREMENTS: Applicants must have 52 weeks of professional nursing experience equivalent to the GS-09 to qualify for the GS-11.
