Allergies and current prescription medications will be logged and verification of NPO status and transportation arrangements will be checked.   ~ Discuss with the patient the medications used for the purpose of sedation and/or pain management.   ~ Obtains the patients signed consent form to receive sedation, consent to have the MRI exam performed, and consent to receive intravenous MR contrast agent (Gado) if ordered by their physician. If the patient is a minor, the parent or guardian will sign the form.   ~ Physically obtains written sedation orders from an on-site facility physician after providing him/her with patient’s final screening documentation.   ~ Ensures proper documentation of the on-site facility physician’s sedation orders and witnesses signature on the patient’s scan log (medical record).   ~ Reviews the “Post sedation guidelines/discharge instructions” form with the patient before the medication for sedation and/or pain management has been given to the patient.   ~ Has the patient sign the “Post sedation guidelines/discharge instructions” form before sedation has been given. If the patient is a minor, the parent or guardian will sign the form. Make a copy of the form and send one copy home with the patient and keep the original as a part of the patient’s permanent medical record.   ~ Administers the approved ordered amount of sedation by the on-site facility physician to the patient.   ~ Has patient change from their clothes into a scrub pant and top.   ~ Has patient secure their belongings in the dressing area.   ~ Maintains current knowledge of universal precautions, observes and practices them when necessary.   ~ Maintains knowledge of aseptic technique, observes and practices them when necessary.   ~ Understands methods and is proficient with venipuncture.   ~ Performs venipuncture if exam has been ordered with an MR contrast agent following department policies & procedures for venipuncture.   ~ Escorts patients to the MRI waiting area prior to exam.   ~ Assists in the transport, moving, or transfer of all non-ambulatory patients throughout the MRI department and SRSA facility.   ~ Prepares the exam table with appropriate coils, cushions, straps, accessories and linen for all patient exams.   ~ Demonstrates a working knowledge of the GE MR exam table and it’s safety features. The nurse will be able to safely raise and lower the exam table, and move the exam table from the bore of the magnet to the home position.   ~ Follows state statute(s) and/or where institutional policy permits, prepares, identifies and/or administers MR contrast agents and/or medications as prescribed by a physician or licensed practitioner. (See SRSA MRI Dept “The administering of contrast media and paramagnetic agent policy & procedure”). (See SRSA MRI Dept “Oral sedation policy”).   ~ Assesses all patients upon completion of MRI examinations. If patients have received oral sedation, the assessment will consist of verbal communication with the patient to assess the level of alertness. The “Post sedation guidelines/discharge instructions” form will be reviewed again with the patient prior to discharge.   ~ Obtains follow up appointment for patients with ordering physician (if the patient is from an SRSA physician).   ~ Properly discharges patients after MRI examination.   ~ Proficient with EMR (electronic medical record) scanning in MRI reports to the appropriate chart and tasking reports to the ordering physician’s basket.
