Coordinates comprehensive health care and community-based services, home visits, and assessments. Develops, implements, coordinates, and evaluates health care plans in an outpatient environment; collaborates with other health care providers and community service agencies to coordinate care. Essential Job Functions: Perform skilled nursing services under the supervision of a Registered Nurse, according to policy, including such duties as the following: 1. Administer prescribed medications and treatments according to law and as permitted within the scope of the individual?s license. 2. Perform nursing care according to the needs of the patient and as indicated in the written plan of care and agency policies and procedures. 3. Inform the physician and other personnel of changes in the patient?s condition and needs. 4. Prepare clinical and/or progress notes and incorporate them into the clinical record at least weekly. 5. Teach self-care techniques to the patient and/or family. 6. Develop plans of care. 7. Participate in in-service programs. 8. Work under the supervision of a registered nurse. 9. Observe, record, and report to the immediate supervisor the general physical or mental condition of the patient. 10. Assist the physician and registered nurse in performing specialized procedures. 11. Assist in development of the plan of care. 12. Prepare equipment and materials for treatments, including sterilization and adherence to aseptic techniques. 13. Assist the patient with activities of daily living and encourage appropriate self-care. 14. Perform complex would care if in-service is documented for specific procedure. 15. May perform routine venipuncture (phlebotomy) if written documentation of competency is in personnel record. Competency must be evaluated by an R.N. even if L.P.N. has completed a certification course. 16. Consult the RN in making judgments and decisions regarding care and services rendered. 17. Submit a schedule of anticipated visits to the DCS on a weekly basis. 18. Calls client/patient and schedules appointment for clinical visit. Must arrive in a timely fashion to client/patients home. If unable to do so, must call client/patient in advance, arrange different appointment times, and document on daily paperwork. The L.P.N. shall not: 1. Access any intravenous appliance for any reason or assume responsibility for IV therapy in the home. 2. Perform supervisory aide visit. 3. Alter the plan of care, make initial assessment visit, prepare the recertification, make aide assignments, function as a supervisor of the nursing practice of any registered nurse, direct and supervise quality assurance programs, or act as a consultant to any home health agency. 4. Essential Job Requirements: EDUCATION Current LPN license in the state where services will be provided. EXPERIENCE One year clinical experience in home health preferred with at least one year clinical experience required. REQUIRED SKILLS Current CPR Certification PREFERRED SKILLS Knowledge of home health federal, state, accreditation regulations. Demonstrates qualities of ethics and integrity, dependability and reliability. Demonstrates excellent clinical and documentation skills.
