Job Title: Care Management Coordinator (Clinical LPN) Company: MTM Inc Location: Saint Paul, MN Description: The Care Management Coordinator (clinical) is responsible for performing Level of Need (LONs) assessments for Special Transportation Services (STS) to ensure individuals receive the most appropriate level of service to their non-emergency medical appointments based on their medical condition.
