The Care Management Coordinator (clinical) is responsible for performing Level of Need (LONs) assessments for Special Transportation Services (STS) to ensure individuals receive the most appropriate level of service to their non-emergency medical appointments based on their medical condition. RESPONSIBILITIES: •Perform LON assessments via phone/fax with authorized party•Completed LON’s require a determination within 5 business days of receipt•Determinations are appropriate based on direction dictated by Department of Human Services (DHS) and recipients level of need•Follow Reconsideration and State Fair Hearing Appeal process•Appropriate documentation in MTM and DHS systems•Mail approval and denial letters within two business days of the determination•Save and label LON assessments, medical records, letters in the shared drive for a period of 10 years•Available to take calls received to queue and adhere to identified phone compliance standards•Follow assigned break schedule to ensure proper phone coverage•Provide excellent customer service•Adhere to HIPAA guidelines•Follow all URAC and MTM/MNet policies and procedures•Assist in reviewing and updating Work Flows, LONs, and correspondence•Act as a clinical resource for staff•Assist in training of MNet staff•Verify recipient eligibility and living arrangement code prior to performing LON•Compare Medical Necessity Reviews for STS Prior Authorization requests received from transportation providers transporting recipients over 40 miles one-way•Complete email requests from other MTM departments•Coordinate decision of Level of Need with supervisor and appropriate state representatives as needed•Educate recipients on STS/ATS and other non-emergency transportation services available to them•Develop a strong relationship with facilities, case workers and discharge planners•Regular attendance is required•Other duties as assignedQUALIFICATIONS: •Bachelors degree required in Nursing•Licensed Practical Nurse required •Registered Nurse preferred•Minimum of one year experience in customer service required•Previous experience working with Medicaid and or State programs preferred•Experience working for a managed care or healthcare provider preferredCandidates must apply on line at:https://home.eease.adp.com/recruit/?id=495669
