The PEAT nurse uses critical care nursing expertise under the auspices of approved protocols to assist staff nurses with appropriate assessment and treatment of clinically deteriorating patients and rounds on Adult Nursing Units in a consistent manner. The PEAT nurse responds to PEAT calls within 5 minutes and in collaboration with the PEAT Respiratory Therapist. The PEAT nurse contacts the attending Physician, Hospitalist, or Intensivist, as appropriate, and will remain with the patient if the attending physician orders a transfer to another unit, until care is assumed by the staff nurse on that unit. : Facilitates the uninterrupted continuing medical care of the patients on POW or in Ketchikan by efficiently coordinating resources necessary for the transition from inpatient facilities to home care, providing 'skilled' home care services for patients referred from inpatient facility, clinics, family members or the respective patient with an MD order until goals are met or care is transferred to another provider. All duties are performed in a manner consistent with the standards and values of KGH. POSITION REQUIREMENTS 1. Education: Graduate of an accredited nursing program. BSN preferred. 2. Certification: Current Registered Nurse licensure in the State of Alaska required. Valid Alaska Drivers License required. CPR required. 3. Experience: A minimum of 2 years nursing experience with an additional year of experience in home health and/or discharge planning required. Commensurate with educational preparation demonstrates knowledge and ability to apply age-specific principles of growth and development and life stages to meet each patient's needs for nursing care. The ability to work in a fast paced, high volume environment is required. 4. Interpersonal: Must have a mature, stable personality and be able to be caring for patients. Must have cooperative working skills, leadership skills, interpersonal skills and the ability to work effectively with a wide variety of patients, families and staff. Must be well-organized, flexible and able to function independently. The ability to demonstrate a courteous, caring and understanding attitude toward patients, families, coworkers, volunteers, visitors and physicians is required. 5. Physical: Must have ability to independently move, lift and position patients weighing up to 200 pounds and assist others in moving, lifting and positioning patients weighing over 200 pounds. Must be able to bend and stoop frequently. Must be able to communicate verbally with staff, patients, families and physicians; read and write text; read and interpret applicable diagnostic screens; hear and interpret Doppler and stethoscope sounds; use senses (vision, hearing, smell touch) to recognize changes in patient's condition. Must be able to perform CPR in emergency situations. ESSENTIAL DUTIES ' Efficiently coordinates the flow of information between hospital and home care setting ensuring a high level of quality care can be maintained. ' Administers nursing care with professional excellence and genuine kindness and respect toward the patient. ' Functions as Case Manager for designated Home Health patients by developing and implementing the Home Health Plan of Care with the patient and MD unless specified as 'relief' or 'visit nurse' only status. ' Uses nursing process in the delivery of nursing care in a manner consistent with KGH and department policies and procedures. ' Identifies problem solving within authority and ability; promptly (within 24 hours) notifies supervisor of situation beyond scope of authority. ' Uses nursing process in the delivery of nursing care in homes demonstrating effective problem-solving and professional judgment. ' Work cooperatively with other Home Health clinicians and patient caregivers. ' Functions effectively as part of the home health care team with professional excellence. ' Enters data in Home Health Electronic Medical Record system, completes paperwork, specific projects, reports and other duties required for licensure and reimbursements requested by the supervisor to accomplish goals of the home health department ensuring an efficient operative department. ' Communicate routinely and as needed with HH staff at KGH Home Health ' Participates in the ongoing Quality Improvement (QI) Program that is organized and responsive to employees so that quality patient care is assured. ' Performs other duties as directed in a cooperative and efficient manner. ' Professionalism, clinical motivation and participation in continuing education are pursued to ensure employee effectiveness. ' Personal dress, grooming and good housekeeping practices are maintained promoting a professional appearance. ' Travel to Ketchikan for orientation/education/job updates as needed.
