Job Title: RN Referral Coordinator, Home Health, Per Diem (Help as Needed), INTERNAL APPLICANTS ONLY Company: St. Peter's Hospital Location: Helena, MT Description: JOB SUMMARY: Registered Nurse who does initial screening on referrals for Home Health nursing, aide, social work and therapy services.  Completes registration, insurance verification and assigns patient to appropriate Case Manager for intake.  Acts as public relations liaison with referral sources, physicians and other hospital departments. Takes orders for changes in care plans and communicates with nursing staff, aide scheduler, and Clinical Manager continually during the day.  Referral Coordinator is also responsible for working with Case Managers to assure visit orders are followed and appropriate visits are scheduled.  Referral Coordinator will assure that Case Managers schedule Case Conferences when appropriate.  Referral Coordinator works closely with the account representatives responsible for billing home health services, to assure they have accurate information to obtain reimbursement.  Referral Coordinator reports to the Clinical Manager and Director. The Referral Coordinator acts as a resource for all home health and hospice staff and works closely with the Clinical Manager, Clinical Analyst and Director as an effective administrative team.KNOWLEDGE/EXPERIENCE: At least two years of acute care or home health/hospice nursing experience required. Understanding of regulatory and billing requirements related to HomeLink programs.  Ability to deal with multiple different referral sources and insurance representatives, has excellent customer service skills and proficient with computers/systems. EDUCATION: Graduate of accredited school for registered nursing.
