**IMMEDIATE OPENING**

*$1,500 SIGN ON HIRING BONUS**

**LOCAL SEATTLE AREA CANDIDATES ONLY, OUT OF STATE CANDIDATES WILL NOT BE CONSIDERED AT THIS TIME**

The Client Care Manager is responsible for the assessment and plan of care for clients as well as the mentorship and supervision of our caregivers.. Daily tasks include: home care assessments, supervision of caregivers, and evaluation of care needs, advocacy, and collaboration with other members of the care team.

FUNCTIONS

•Conducting assessments for new clients
•Creating care plans for new clients
•Performing supervisory visits for caregivers to assure they continue to meet Fedelta's standards
•Advocate for clients as needed

-Training New LPNs on Pediatric Cases and geriatric cases

-Assisting with Care Management Duties

-Assisting with developing care plans and collaborating with care teams (doctors, social workers, etc).

-Attending trainings as necessary and evaluating LPNs and CNAs on your cases

QUALIFICATIONS

  • *LPN Licensure Required
  • Ventilator and Trach experience REQUIRED
  • 2-3 years of Hospital or Direct Patient Healthcare Experience
  • 1-2 years of Pediatric Population experience required
  • Management experience is a big plus, but not necessary

•Home Care, Senior Care, or Care Management experience preferred
•Geriatric Certificate preferred
•Background in scheduling and care coordination helpful OR
•1-2 years of experience working in an acute care setting
•Able to successfully pass mandatory background checks
•Valid driver's license, reliable automobile, and valid automobile insurance

WORK ENVIRONMENT & PHYSICAL REQUIREMENTS

•Must have a valid driver's license, current car insurance and reliable transportation to and from the Greater Seattle area
•Must be able to read, write, and communicate fluently in English
•Must be able to use a desk top or laptop computer and related peripheral and office equipment
