Registered Nurse - Home Visit Concierge Service (Initial & Reassessment Visits) Reports To: Supervisor
Job Type: Per Diem / Contract (RN Agency)
Location: Various Client Homes, Mt Laurel, Medford, Marlton,
Position Summary: We are seeking an experienced Registered Nurse (RN) to join our Home Visit Concierge Service on a per diem/agency basis. This role requires nurses who work well in fast-paced environments and can adapt as patient needs and operations change. The RN will conduct initial assessments and reassessments in the patient's home, providing care that ensures safety, wellness, and continuity.
Key Responsibilities:
Conduct comprehensive initial nursing assessments in accordance with agency policies, state and federal regulations, and payer requirements.
Perform reassessments on a routine schedule or when changes in patient condition are observed or reported.
Accurately document clinical findings, assessments, interventions, and follow-ups using the agency's EMR system (software).
Collaborate with interdisciplinary teams to ensure coordinated care plans and address patient needs in real time.
Identify and report significant findings or safety concerns and initiate appropriate follow-up actions.
Educate patients and families on condition management, medications, and health maintenance strategies.
Maintain professional etiquette and cultural sensitivity during home visits.
Required Qualifications:
Experience in home health, emergency services, urgent care, and/or concierge nursing.
Ability to work independently in patient homes and make clinical decisions.
Reliable transportation and willingness to travel within assigned service areas.
Strong organizational and communication skills.
BLS Certification (Required).
Comfort with mobile technology and EMR systems.
Rate: $80.00 per hour for home visit
Rate: $40.00 per wellness phone call
