Qualifications:

• Graduate of an accredited nursing program

• Minimum of one (1) year of recent experience in acute care or home health care

• Current LPN licensure in the state of Oregon

• BLS/CPR certification

• Valid drivers license and auto insurance

• Strong verbal and written communication skills

• Ability to meet physical demands including standing, lifting, and bending

Preferred:

• Associates degree in nursing

• Experience in home health care

Key Responsibilities:

• Provide patient care under the supervision of a clinician, ensuring that care plans are followed accurately

• Monitor and document vital signs, medical histories, and patient conditions

• Educate patients and caregivers on health management and home care techniques

• Coordinate with other healthcare professionals (HHA, OT, PT, MSW, ST, etc.) to ensure comprehensive patient care

• Maintain accurate clinical documentation according to the care plan

Work Details:

• This is a traveling position, requiring a valid drivers license and a reliable vehicle

• Flexible schedule: 5 days per week with one weekend per month on call

• Work remotely with opportunities for field-based care

• Shifts typically do not require late nights
