The Chronic Disease Case Manager is responsible for assessing, educating, supporting, and following up with the specific patient populations with chronic diseases. The Chronic Disease Case Manager will work collaboratively with the entire multidisciplinary team, patients, families, post-acute providers, and outpatient vendors to ensure safe, timely, and efficient discharge to the next level of care while preventing readmissions.

II. Major Responsibilities:

1. Meets with patients that carry a chronic disease diagnosis to assess their level of understanding, need for education, and level of home or community support.
2. Educates patients and care givers regarding their specific chronic disease, including the need for adherence to specific diet, medications, and follow up care.
3. Makes referrals to home care agencies to support this patient population in the community.
4. Collaborates with the Cardiology staff regarding risks of readmissions and intervenes to prevent readmissions whenever possible.
5. Assists in data collection for the updating of metrics to achieve productivity targets, eliminate errors, and deliver excellent customer service.
6. Support Nursing staff by providing education to patients with chronic disease.
7. Partner with multidisciplinary, post- acute, and remote monitoring teams to improve patient outcomes.
8. Acts as support for other duties not listed as assigned.

III. Position Qualifications: The applicant should have strong case management or discharge planning background with emphasis on readmission prevention. Demonstrated ability to provide effective patient/care giver education. Proven ability to work independently.

License/Certification/Education:

Required:
1. Massachusetts RN license in good standing
2. BSN from an accredited school

Preferred: ACM or CCM

Experience/Skills:

Required:
5 years of Case Management or Discharge Planning experience
Focus on Readmission Reduction
Chronic Disease Patient Education
Critical thinking skills
Excellent written and verbal skills
Computer literate various EMRs and Microsoft Office

Preferred: Experience with Report Creation

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies, including age-specific competencies and their measurements, will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.

IV. Physical Demands and Environmental Conditions

Work is considered active, requires sitting, standing, and walking. Position requires work indoors in a patient care environment and office setting. The position requires use of a computer and telephone. Position uses printed educational materials.
