Position Summary
The PC Nurse Manager is responsible for coordinating clinical care of residents and implementing processes to ensure quality care and safety of all Personal Care residents. The role is key to educating and supervising Med Techs in providing quality care, ensuring accurate medication administration and adhering to Department of Human Services regulations. This role will also ensure appropriate staffing levels are met and maintained. This role will also perform other duties as required.
Essential Duties and Functions
- Monitor and continually assess PC residents to obtain information to develop appropriate interventions.
- Assess residents upon admission or readmission, annually or upon significant change to develop or update support plan as required. Attend and participate in RASP meetings with residents and/or families as scheduled.
- Update medical records as required by appropriately documenting changes in resident condition, incidents and follow up investigations, re admissions, any changes related to physician orders, and any other relevant updates as required by our policies and procedures.
- Collaborate with interdisciplinary team members including physicians, therapists and caregivers to develop and implement individualized support plans.
- Effectively communicate with residents, families and personal care staff regarding resident changes related to medical or behavioral concerns and changes in level of care.
- Review residents' support plans and preferences to ensure that they remain current and reflective of their evolving needs and desires.
- Make rounds to see residents; making observations and taking appropriate interventions as needed.
- Respond to resident emergencies, assess resident's current condition and determine appropriate interventions based on Personal Care policies and procedures.
- Timely completion and submission of DHS reportable incidents.
- Ensure that residents are aware of the services available to them, encourage them to participate in life enrichment activities, and provide staff support to enable them to get to and from activities.
- Manage PC Med Techs' care assignments and ensure all duties are completed timely and as required; will have on call responsibilities for employee and resident issues and concerns.
- Serve as a resource and mentor to PC Med Techs and other staff members, offering guidance and assistance as needed.
- Ensuring adequate staff coverage to meet proper ratios, PPD and acuity of residents according to daily census.
- Conducts resident incident investigations; communicates verbally and in writing the results of the investigations, outcomes, and recommendations for the plan of action.
- Provides leadership and direction to the department staff including hiring, training and performance management.
- Makes informed hiring decisions by ensuring candidates are the appropriate fit for the department from a technical, strategic and cultural standpoint.
- Documents individual performance metrics, behaviors, discipline and performance ratings and delivers feedback in an appropriate, courteous and professional manner.
Qualifications
Qualifications include:
- Previous experience in a leadership role within a senior living or healthcare setting, with a strong understanding of personal care services and regulations.
- Minimum 2 years nursing in a senior living environment preferred
- Knowledge of DHS personal care regulations preferred
- Supervisory experience preferred
Education:
- Graduate of an approved school of nursing
Licensure/Certification:
- Current LPN license, current CPR certification
