Registered Nurse – Home Dialysis

We are seeking a dedicated and experienced Home Dialysis RN to join our dynamic care team. In this role, you will provide high-quality, patient-centered care for individuals undergoing dialysis treatments at home. This position is ideal for a registered nurse who is passionate about education, empowering patients, and delivering personalized care outside of the traditional clinical setting.

Key Responsibilities:

• Coordinate and manage care for a caseload of home dialysis patients (Peritoneal and/or Home Hemodialysis).

• Perform comprehensive assessments and develop individualized care plans in collaboration with the interdisciplinary team.

• Educate patients and their home partners on dialysis procedures, equipment, and care protocols in a structured training environment.

• Monitor patient response to treatment and adjust care plans as needed.

• Conduct regular home visits to assess treatment environment, provide support, and ensure adherence to safety standards.

• Respond to patient concerns and participate in on-call rotation for urgent care needs.

• Document clinical data accurately and in a timely manner.

Qualifications:

• Current RN license in the applicable state.

• Minimum of 1 year of nephrology nursing experience within the last 2 years.

• At least 1 year of experience in home dialysis (PD and/or home hemodialysis) strongly preferred.

• Graduate of an accredited nursing program.

• BLS certification

• Strong clinical judgment, critical thinking, and communication skills.

• Ability to work independently and manage a flexible schedule that may include travel, on-call, weekends, or holidays.

Working Conditions:

• Frequent travel to patient homes and other care sites as needed.

• Requires ability to lift/move equipment up to 30 lbs and assist with patient mobility.

• Combination of clinical and administrative work (home visits, patient education, charting).
