Our Diabetes Nurse Case Manager is someone who has a passion for educating patients on diabetes, enjoys 1-1 and group settings and is looking to help patients improve their lives through short- and long-term lifestyle adjustments, while maintaining cultural sensitivity.
Job Duties:
- Engage in 1-1 appointments to optimize patient treatment plans.
- Facilitate group classes.
- Actively participate in community outreach, including attending occasional events.
MINIMUM QUALIFICATIONS:
- BSN Degree.
- Current RN License.
- Two (2) years of nursing experience.
- Current BLS certification.
PREFERRED QUALIFICATIONS:
- Spanish speaking.
