HAPI is seeking an FFS LPN to join our team. The FFS LPN will report to the Nurse Supervisor.
JOB SUMMARY: Consistent with H.A.P.I.'s mission, policies, and governing scope of practice, and under the supervision of a Registered Nurse, will
assist with the Home Health Aide Training Program, to ensure the competency and certification of trainees.
DUTIES AND RESPONSIBILITIES:
1 Assist with the Home Health Aide Training Process and Certification.
a. Welcomes the trainees to the HHATP.
b. Ensures that the trainees sign the attendance sheet.
c. Communicates with the nursing supervisor and reports lateness and/or absences.
d. Ensures that the trainees understand the length of the program and their role and responsibilities.
e. Hands out the Curriculum and Reading Requirements.
f. Hands out the Text/workbooks.
g. Keeps a track of books purchased.
2 Maintains trainees records based on the local, state, regulations as well as .
accreditation standard.
a. Ensures that the attendance sheets are kept in a binder.
b. Ensures that trainees written competency, Supervise Practical Training and Certification are in the trainees records.
- Utilizes knowledge of diagnosis and clinical skills to appropriately assist nurse supervisor with the training of health related task.
a. Assess trainees strengthens and weaknesses.
b. Provides time with trainee for additional assistance/consultation.
c. Assists trainee in health related task in a laboratory setting observation.
4 Establishes realistic goals and methodology for goal achievement with the nursing supervisor and trainee.
a. After consulting with nursing supervisor, will identify realistic goals and ways to achieve them in collaboration with the trainee, Director
of Nursing and Director.
b. Ensures that the trainee understands what is required to achieve identified goals.
c. Demonstrates a high degree of commitment to customer service and quality care provision.
5 Interacts with workers providing personal care services when providing visits to a client's home.
a. Makes field visits to observe clinical practice as well as compliance with established plan of care and provide supervision.
b. Provides orientation to newly competent assigned and/or replacement HHA/PCA.
c. Communicates with HHA, on each nursing visit to ensure that she/he understands her/his role in the client's home.
d. With the guidance of a RN, maintains an updated plan of care for the aide and reviews with aide and client/family.
e. Reports and observes aide's rapport with client and family. .
f. Assists HHAs' competency through established protocol via in-home visits as well as laboratory setting observations.
6 Prepares written documentation and summaries of client's progress as needed for continuation of home health services.
a. Completes a nursing progress note for each visit rendered within established parameters
b. Documents all telephone conferences and team conferences in a timely manner.
c. Promptly reports and documents incidents or allegation of client abuse, neglect, mistreatment, errors in treatment or medications,
accidents, incidents and complaints.
d. Completes all other required documentation in a timely manner (i.e. nurse Note).
7 Participates in Performance Improvement activities:
a. Reviews complaints and intervenes as per agency policy.
b. Reviews incidents/accidents and intervenes as per agency policy.
8 Attends appropriate meetings, case conferences, in-services, etc.
a. Attends and participates in all mandatory meetings and in-services.
b. Identifies the need for, plans, and participates in administrative case conferences as necessary
c. Participates in performance improvement clinical reviews and other committees as designated.
9 Performs other duties as assigned.
a. Takes every opportunity to educate prospective clients, trainees, physicians, and community resources of HAPI's services.
b. Participates in health fairs or community programs as required.
c. Develops relationships with prospective and/or actual referral sources.
QUALIFICATIONS,SPECIALIZED SKILLS AND COMPETENCIES:
- Licensed LPN in NYC
- Demonstrates knowledge and clinical expertise within the scope of practice of a Licensed Practical Nurse practicing in New York State.
- Demonstrates an ability to interact with paraprofessionals and identify potential problems.
- Demonstrates knowledge of New York State Department of Health regulations for Licensed Home Care Service Agencies and Joint Commission Accreditation standards.

- Ability to plan and conduct duties in accordance with age appropriate capabilities and needs of the elderly and adult population.
PHYSICAL DEMANDS:
May encounter prolonged standing, walking, frequent lifting, and positioning clients, pushing and/or transferring clients, lifting supplies and equipment, frequent twisting, stooping, bending, squatting, and climbing stairs. Can lift up to 50 lbs. unassisted and up to 325 lbs. utilizing an assistive device in the home environment. In the office, lifting is limited to files and records not to exceed 10 lbs.
