Coordinate the overall interdisciplinary plan of care for a patient, serving as the liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Responsible for scheduling, organizing and managing all aspects of a patient s healthcare maintenance and treatment. Their duties include helping patients complete paperwork, expedite patient care referrals, communicating with a healthcare team about a patient s treatment plan and educating patients about resources and options for managing their health.

Uphold the organization s vision, mission, and corporate values. Demonstrate understanding of and compliance with organization s policies, procedures, code of conduct and work rules.

Essential Functions:

Promote patients' independence by continuing the plan of care established by providers and teaching patients and families to understand conditions, medications, and self-care skills

Network with providers and agencies to provide ongoing follow-up and care services,

Coordinate patient appointments for outpatient services, specialty clinics, and external clinics, and confirm travel arrangements

Obtain medical records from other facilities and providers and verify records are complete before forwarding to Health Information Management

Process patient referrals from NSHC and Specialty providers and track through Cerner Referral Management

Place orders per provider request for labs, radiology, travel, and referrals

Plan and organize specialty clinics and other events in support of healthcare promotion

Act as a relief LPN in the Outpatient Clinic when necessary

Perform other duties as assigned
