Responsibilites:
- Supervise residents who self-administer medication while following company and regulatory guidelines
- Maintain resident records regarding incidents; Observe residents for safety and reports any and all changes in resident condition effectively, accurately, and in a timely manner to LN on duty
- Assisting residents with ADL tasks including but not limited to: assisting with bathing, grooming, toileting, dressing, oral care, ambulation, transfers, serving and assisting with meals, feeding, housekeeping, bed making, and laundry
- Always observe and uphold resident rights; assist residents to and from meals; Relate to residents in a courteous, empathetic, and cooperative manner
- Must be willing to work with and provide assistance to other staff
- Assist Director of Resident Services with resident documents, chart organization and other duties as needed.
- Communicates with physicians, via fax and telephone
- Keep environment safe and clean for residents
- Perform other related duties as assigned by DRS
- Carry and posses a working knowledge of community communication system.
- Observes for and reports resident problems and reports such problems to Director of Resident Services.

- Must be neat in appearance, pleasant personality, be congenial to fellow team members, residents, families and the public in general. Must be capable of using own judgement and initiative in daily duties.
- Always demonstrates safe working practices.
- Maintains a professional image to residents and fellow co-workers in words, actions and in appearance
- Always maintain confidentiality and privacy of our residents and staff.
- Must possess an upbeat, positive, flexible, and enthusiastic personality and be congenial with fellow employees, residents, and the public at large.
- Must be an honest and dependable individual.
- Must maintain proper hygiene and be professional in dress and presentation.
- Must be a mature individual. Must possess the ability to multi-task in a fast-paced environment and perform a variety of tasks, often changing assignments on short notice.
- Must be detail and teamwork oriented and have strong organizational skills, as well as the ability to prioritize and meet deadlines.
Qualifications:
- Must be Drug free
- Must be able to pass Criminal Background Check and TB test.
- Must be eighteen (18) years old.
- Ability to read, analyze, and interpret charting standards, physician orders, or administrative regulations.
- Assisted Living and Memory Care regulations familiarity
- Ability to prioritize, organize and delegate assignments.
- Must attend required in-services & education to further education of skills and community requirements.
- Ability to communicate clearly and effectively, verbally and in writing in standard English.
Carries the following Certifications (current):
- CNA or HCA License
- CPR/First Aid (in person, no online certificates accepted)
- Nurse Delegation Core
- Diabetes
- Mental Health
- Dementia
- Blood Born Pathogen
- Food Handlers
The employee shall be responsible for maintaining the integrity of Fields Senior Living
Physical Demands and Requirements:
- Maintain ability to lift 50 pounds; to stand or walk throughout the vast majority of the workday; and to frequently push, pull, bend, twist and reach.
- Must be willing to be active and on feet during entire work shift.
- Involves light to heavy physical activity performing non-strenuous and strenuous activities.
- Must have sufficient manual dexterity to work with the fingers and to reach/hand items.
