We are seeking a compassionate and motivated Licensed Practical Nurse LPN to join our Home Health team.

What Youll Do
Provide skilled nursing care in accordance with physician orders and patient care plans
Review and follow the Plan of Care during each patient visit
Monitor patient conditions and communicate changes with the RN Case Manager and care team
Educate patients and caregivers on disease management and self-care techniques
Document patient care accurately and timely
Collaborate with physicians, clinicians, and interdisciplinary care teams
Support efforts to reduce avoidable hospitalizations and improve patient outcomes
Deliver compassionate care and exceptional patient experience
What Youll Need
Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
Current Indiana LPN license
Minimum of 1 year of nursing experience in a clinical care setting
Home Health or Hospice experience preferred
Reliable transportation, valid drivers license, and current auto insurance
Ability and willingness to travel within assigned territory
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
