Role Overview

As a Home Health RN, you are the clinical lead for your pients - managing the plan of care, coordining across disciplines, and driving outcomes from admission through discharge.

Youll combine advanced assessment skills, care planning, and pient educion to reduce hospitalizions, improve recovery, and support long-term independence home.

Wh Youll Do

• Conduct comprehensive in-home assessments to evalue clinical stus, risks, and goals of care
• Develop and manage individualized care plans in collaborion with physicians and care teams
• Deliver skilled nursing care, including medicion management, trements, and interventions
• Serve as the case manager, c oordining care across therapists, aides, and physicians
• Monitor pient progress and make clinical decisions based on changing conditions
• Educe pients and families to support disease management and independence
• Coordine care transitions and discharge planning to reduce readmissions
• Maintain timely, accure documention in the EMR
• Supervise and support Home Health Aides as needed
• Participe in on-call rotion as approprie

Wh You Bring

• Active RN license (or compact eligibility) + CPR certificion
• Gradue of an accredited nursing program
• Minimum one (1) year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred)
• Strong clinical judgment and ability to think independently
• Excellent communicion skills and pient-centered approach
• Ability to manage a caseload and prioritize effectively
• Proficiency with EMR systems
• Valid drivers license and reliable transportion

Wh Sets You Apart

• Experience in home health or post-acute care
• Experience with case management and interdisciplinary coordinion
• BSN
• Familiarity with Homecare Homebase (HCHB)
