Join our team as a Remote Case Manager RN and play a critical role in guiding members toward better health outcomes through personalized care coordination.What You'll DoConduct comprehensive clinical assessments and develop individualized care plansCoordinate inpatient reviews and discharge planningAdvocate for members and connect them with appropriate services and resourcesMonitor care plans and adjust strategies to ensure optimal, cost-effective outcomesUse motivational interviewing and coaching to support behavior change and engagementIdentify quality of care issues and contribute to program improvementsDeliver exceptional member service in a remote environmentWhat You Bring - REQUIREDActive, unrestricted RN license (with ability to obtain multi-state licensure paid for by the company)3+ years of acute care RN experienceStrong clinical judgment and decision-making skillsExcellent communication, organization, and multitasking abilitiesProficiency in Microsoft Office toolsPreferred QualificationsCompact RN licenseCertified Case Manager (CCM)Case management or managed care experienceTelephonic care or customer service experienceOncology care experienceScheduleMonday-Friday, 8:00 AM - 4:30 PM (local time)Occasional evening rotation (10:30 AM - 7:00 PM)Rotational evenings, weekends, and holidays based on department needsEducationAssociate Degree in Nursing required (BSN preferred)Make an impact from home-help members navigate their care, improve outcomes, and live healthier lives. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
