We are seeking a Travel Licensed Practical Nurse (LPN) to join our home health team supporting patients across Wicomico County. This role focuses on delivering high-quality, patient-centered care in a home setting while ensuring timely and accurate documentation.The ideal candidate is dependable, detail-oriented, and passionate about providing compassionate care in a home health environment.This job will have the following responsibilities:* Deliver skilled nursing care to patients in their homes in accordance with care plans and physician instructions* Monitor patient health status, including vital signs and symptom assessment* Administer medications and treatments within the LPN scope of practice* Provide education to patients and caregivers on treatment plans and health management* Complete accurate and timely documentation of patient visits, treatments, and outcomes* Maintain up-to-date patient records in compliance with organizational and regulatory standards* Ensure all care provided aligns with company policies and clinical guidelines* Collaborate with physicians, RNs, and interdisciplinary care teams to support patient care plans* Communicate changes in patient condition promptly to appropriate stakeholders* Participate in care planning and follow-up activities* Ensure delivery of high-quality, compassionate care that supports positive patient outcomes* Maintain patient confidentiality and dignity at all times* Ad to infection control and safety protocols
Qualifications & Requirements:
* Prior experience in home health care
* Ability to work independently and manage patient visits efficiently
* Strong organizational and time management skills
