As a Heart Failure ED Nurse Coordinator, you will support fellow caregivers in delivering exceptional patient care from initial evaluation through discharge and follow-up. In this role, you will interact directly with patients, families, and visitors, helping enhance the patient experience while providing education and support related to heart failure management and treatment plans. This position focuses on evaluating lower-acuity heart failure patients in collaboration with Emergency Department providers and cardiologists to ensure patients receive comprehensive education, medication guidance, and self-management resources that may not otherwise be fully addressed during an ED visit. Responsibilities include coordinating outpatient diuresis plans, arranging urgent clinic follow-up, ensuring medications and discharge orders are accurately entered, and communicating care plans prior to discharge.

A key aspect of this role is helping patients safely transition home with the appropriate support, education, and follow-up care in place to reduce avoidable admissions and readmissions while promoting continuity of care and improved patient outcomes. A caregiver in this position works Monday thru Friday from 8:00 a.m.-5:00 p.m. (No weekends or holidays)

A caregiver who excels in this role will:

  • Provide nursing care to patients undergoing diagnostic or therapeutic services to hospitalized patients in various healthcare settings.

  • Assess patients, complete nursing assessments, document findings in patient records and notify physicians of abnormal findings.

  • Perform telephone triage and call rotations.

  • Perform nursing procedures and administer treatments and medications.

  • Establish care plans in collaboration with patients, families and the health care team.

  • Coordinate ongoing care, identify discharge needs and facilitate discharge planning.

  • Evaluate, initiate and maintain standards of care.

  • Educate patients and families.

  • Provide standardized coordination and support for Acute Decompensated Heart Failure (ADHF) presentations and high-risk heart failure patients across multiple care settings.

  • Support Emergency Department teams with triage and disposition planning for inpatient admission, observation admission, outpatient treatment arrangements, and follow-up care coordination.

  • Coordinate transitions of care for patients discharged home with CCC or UD IV furosemide arrangements, including education and heart failure follow-up scheduling.

  • Assist with home management plans involving subcutaneous furosemide kits, patient education, and ongoing heart failure follow-up.

  • Facilitate regional inpatient admissions through the established MedHub ED program and support potential Hospital at Home initiatives as developed.

  • Ensure rapid access to heart failure and advanced heart failure clinic follow-up appointments, particularly within 48 hours of Emergency Department or hospital discharge.

  • Coordinate among multidisciplinary teams including HVTI clinicians, CVM schedulers, CCC, UD, pharmacy, and other care teams to reduce delays, bottlenecks, and missed high-priority follow-up appointments.

  • Promote efficient communication and continuity of care to improve patient outcomes and support timely access to heart failure services.

Minimum qualifications for the ideal future caregiver include:

  • Graduate of an accredited School of Nursing (Diploma, ADN, BSN program)

  • Current state licensure as a Registered Nurse (RN)

  • Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross

  • Advanced Cardiac Life Support (ACLS) through American Heart Association (AHA) Certification within six months of hire

  • One year of recent experience as an RN

  • OR Licensed RN with at least 2 years of LPN experience within the same Cleveland Clinic procedural department may substitute for required RN experience

Preferred qualifications for the ideal future caregiver include:

  • Two years of recent experience as an RN

  • Bachelor of Science in Nursing (BSN)

  • Experience in Cardiology, Emergency Medicine or the ICU

  • Strong heart failure knowledge

Physical Requirements:

  • Requires full range of motion, manual and finger dexterity and eye-hand coordination.

  • Requires standing and walking for extensive periods of time.

  • Requires corrected hearing and vision to normal range.

  • Requires some exposure to communicable disease or bodily fluids.

  • Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.

  • Physical Demand requirements are in excess of those for Light Work.

Personal Protective Equipment:

  • Follows Standard Precautions using personal protective equipment as required for procedures.

Pay Range

Minimum hourly: $35.50

Maximum hourly: $55.60
