Position Summary
As an LPN in our Primary Care practice, you will play a vital role in delivering preventive care, managing chronic conditions, and ensuring a seamless patient experience. Working within the Nursing Professional Practice Framework, you will provide quality, efficient, and compassionate care to patients across the lifespan.

Key Responsibilities
Partner with providers and care teams to support preventive care and chronic disease management
Room patients, obtain vitals, and document health information accurately in the EMR
Administer medications, vaccines, and treatments per provider orders
Educate patients on health conditions, medications, and follow-up care
Coordinate care, including referrals, labs, and follow-up appointments
Respond to patient inquiries and support a positive, patient-centered experience

Qualifications

What Were Looking For
Required:
Active LPN license in the state of Connecticut
Strong clinical and organizational skills
Excellent communication and patient engagement abilities
Preferred:
Primary care, ambulatory, or outpatient experience
Familiarity with EMR systems
Experience with care coordination or population health initiatives
