We utilize the latest technology to provide an efficient complete plan of care, with user-friendly web-based documentation. Responsibilities and Duties: Perform evaluation tasks, including vital signs and medication review and complete SOC Develops a Plan of Care based on the patient diagnosis and needs. Dress or redress open wounds and assess progress of healing Educate patients and families on home health care strategies and procedures Make recommendations for devices or tools that might improve the patient's quality of life Coordinate with Occupational Therapists, Physical Therapists, Physicians and others involved in the patient's care plan.
