Responsibilities include: Conduct initial nursing assessments and ongoing patient evaluations Develop and implement individualized care plans Provide skilled nursing care according to physician orders and care protocols Maintain accurate clinical documentation and progress notes Supervise, educate, and support caregivers and nursing staff Ensure compliance with HIPAA, state regulations, and federal healthcare standards Collaborate with physicians, families, and interdisciplinary care teams Participate in quality improvement initiatives and staff training programs Monitor patient outcomes and recommend adjustments to care plans as needed Promote a positive patient and family experience through compassionate care Qualifications: Current unrestricted Registered Nurse (RN) license Graduate of an accredited nursing program Bachelor of Science in Nursing (BSN) preferred 3-5 years of clinical nursing experience preferred Previous home health, hospice, case management, or primary care experience preferred Strong nursing assessment and clinical decision-making skills Excellent communication and leadership abilities Ability to work independently and manage multiple patients effectively Reliable transportation, valid driver's license, and auto insurance required Ability to travel locally throughout the service area Knowledge of state and federal home health regulations preferred Ideal Candidates Will Have Experience In: Home Health Nursing Case Management Skilled Nursing Patient Assessments Care Coordination Chronic Disease Management OASIS Documentation Supervisory or Clinical Leadership Roles Apply Today If you're looking for a rewarding RN opportunity where you can make a real impact while enjoying flexibility, career growth, and a supportive team culture, apply today to join BrightStar Care.
