Schedule:

Key Responsibilities:

• Deliver nursing care and skilled clinical interventions in patient homes
• Conduct comprehensive assessments and complete OASIS documentation thoroughly and promptly
• Collaborate with physicians and multidisciplinary care teams to develop and implement individualized care plans
• Perform wound care, medication management, patient education, and disease monitoring
• Communicate effectively with clinical team regarding patient status changes

Desired Qualifications:

• Active RN license
• At least one year of recent home health RN experience
• OASIS documentation experience is required
• Proficiency using HomeCare HomeBase strongly preferred
• Ability to maintain a structured daily schedule and meet visit requirements
• Reliable transportation and a valid drivers license
