Your next nursing opportunity starts here. This contract role is ideal for an experienced Registered Nurse (RN) skilled in home health, with a particular emphasis on OASIS documentation. Youll partner with a professional, supportive care team committed to delivering compassionate, individualized patient care in a home setting.

Schedule:

• 13-week contract with the possibility of extension
• Full-time hours: Monday through Friday
• Structured daily visit expectations
• Probable on-call rotation

Key Responsibilities:

• Deliver direct in-home skilled nursing services with clinical precision and empathy
• Complete comprehensive patient assessments and accurate OASIS documentation within set timelines
• Collaborate with physicians and other clinicians to develop and implement tailored care plans
• Manage wound care, medication administration, patient education, and monitoring for changes in condition
• Maintain thorough communication with the care team regarding patient status and care needs

Qualifications:

• Active RN license
• At least 1 year of recent home health experience as an RN
• Demonstrated proficiency with OASIS documentation (required)
• Experience with HomeCare HomeBase (strongly preferred)
• Reliable transportation and valid drivers license
• Strong organizational skills for daily visit management
