This full-time contract is ideal for a skilled clinician who thrives in a structured environment and is passionate about delivering high-quality patient care in the home setting.

Schedule:

• 13-week full-time contract
• Monday through Friday schedule
• On-call availability may be required

Key Responsibilities:

• Deliver compassionate in-home nursing care, including skilled interventions and monitoring
• Conduct thorough patient assessments and complete OASIS documentation with accuracy and timeliness
• Collaborate closely with physicians and multidisciplinary care teams to create individualized care plans
• Provide wound care, medication administration, patient education, and disease management support
• Communicate effectively with clinical team members regarding patient status and changes

Qualifications:

• Active and unrestricted RN license
• At least 1 year of recent RN experience in home health
• Strong OASIS documentation experience required
• Proficiency with HomeCare HomeBase (highly preferred)
• Ability to efficiently manage a full-time, structured caseload with daily visits
• Reliable transportation and a valid drivers license
