Seeking an experienced Registered Nurse with a passion for home health to provide exceptional care to patients throughout a 13-week, full-time contract. This position offers the chance to make a difference in patients lives through skilled clinical care, accurate documentation, and effective scheduling.

Schedule:

• Full-time, Monday through Friday
• On-call availability may be required

Key Responsibilities:

• Deliver high-quality in-home nursing care and skilled interventions
• Complete patient assessments and timely, accurate OASIS documentation
• Collaborate with physicians and interdisciplinary care teams to implement personalized care plans
• Perform wound care, medication management, patient education, and ongoing disease monitoring
• Communicate changes or updates in patient condition to the clinical team

Desired Qualifications:

• Active RN license
• Minimum one year of recent home health RN experience
• Proven experience with OASIS documentation
• Familiarity with HomeCare HomeBase (highly preferred)
• Able to manage a structured, full-time caseload
• Reliable vehicle and valid drivers license
