LPN Home Health in Harrison, AR

Schedule/Hours:

• Full-time, contract assignment

Key Qualifications:

• Active and unrestricted LPN license
• At least one year of recent experience in home health nursing
• Demonstrated proficiency using HomeCare HomeBase (HCHB) EMR
• Excellent clinical assessment and documentation abilities
• Reliable transportation for in-home patient visits
• Strong organizational skills and the ability to manage a caseload independently

Primary Responsibilities:

• Provide skilled and compassionate nursing care according to individualized care plans
• Monitor patient conditions, update care plans, and document all interventions thoroughly in HCHB
• Administer medications and execute treatments in accordance with state guidelines
• Educate patients and their families on ongoing care management and strategies for disease prevention
