What Youll Do:

• Deliver skilled nursing care in patients homes based on individualized care plans
• Monitor, assess, and document patient progress, making timely updates to care plans
• Administer medications and treatments according to state guidelines
• Educate patients and families on illness management, preventive care, and safe practices
• Maintain thorough and accurate records within HomeCare HomeBase (HCHB) EMR
• Partner with registered nurses, therapists, and other care providers for comprehensive care coordination

Required Qualifications:

• Active LPN license
• At least one year of recent experience in home health
• Proficiency using HomeCare HomeBase (HCHB) EMR
• Well-developed clinical assessment and documentation abilities
• Reliable transportation for patient visits
• Strong organizational skills and the ability to manage an independent caseload
