Home Health Licensed Practical Nurse (LPN) Charlotte

Sunbelt Staffing is seeking an experienced Home Health Licensed Practical Nurse (LPN) for a contract assignment. This position is ideal for a dedicated LPN who is passionate about providing high-quality care to geriatric patients in a home health setting. Candidates must have prior home health experience and be proficient in HomeCare HomeBase (HCHB) EMR.

Responsibilities:

• Provide skilled nursing care to patients in their homes according to care plans
• Monitor and document patient progress, updating care plans as needed
• Administer medications and perform treatments under state guidelines
• Educate patients and families on care management and disease prevention
• Maintain accurate records using HomeCare HomeBase (HCHB) EMR
• Coordinate with RNs, therapists, and other healthcare professionals to ensure comprehensive patient care

Qualifications:

• Active LPN license
• Minimum one year of recent home health experience
• Proficiency in HomeCare HomeBase (HCHB) EMR
• Strong clinical assessment and documentation skills
• Reliable transportation for home visits
• Ability to work independently and manage a caseload efficiently
