An opportunity is available for an experienced Home Health Registered Nurse to provide compassionate, skilled nursing care to patients in their homes. This full-time, 13-week contract offers a field-based role requiring independence, strong clinical judgment, and outstanding patient communication.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
Key responsibilities include:
- Conducting comprehensive OASIS assessments and developing tailored care plans
- Delivering skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management
- Evaluating patient progress, monitoring for changes in condition, and communicating with physicians and interdisciplinary team members
- Educating patients and families on disease processes, care plans, medications, and preventive strategies
- Ensuring accurate, timely documentation in Home Care Home Base (HCHB)
- Coordinating care with physical and occupational therapists, social workers, and home health aides
- Participating in case conferences, staff meetings, and ongoing education
- Adhering to regulatory and agency documentation requirements to uphold high standards of care
Desired qualifications:
- Active, unencumbered RN license for the state of practice
- At least one year of recent home health experience (field experience preferred)
- Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS
- Strong clinical assessment, critical thinking, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Excellent time management and organizational abilities
- Bachelor of Science in Nursing (BSN) preferred
- Prior experience managing a caseload of 2532 points per week
- Familiarity with Medicare and compliance standards for home health
