Job Summary:
The Licensed Practical Nurse (LPN) is a member of an inter-disciplinary treatment team for behavioral health services, either in or outpatient that provides clinical care based on medical/mental health treatment within the scope of Licensed Practical Nurses' protocol under the orders of a medical prescriber. The LPN assists in the management of psychiatric crises, prepares written orders from the prescriber, assists with the management of patient medication needs including but not limited to pharmacy orders, prior authorizations, assisting with management of medication side effects under prescriber orders, maintaining medication administration records, medication storage areas, providing ordered injections, working collaboratively with Crises Services, Emergency Rooms, with Community Providers, and providing patient medication and health education. The LPN assists with care coordination needs through rounds, staffings, external provider communications, and inter-team communications. The LPN supports and provides quality client care through effective assessment, supportive therapies, documentation of client care and findings, treatment planning, and working collaboratively with external and internal treatment team members.
Essential Functions:
- Performs nursing assessments under RN and/or Prescriber supervision or ordered protocols, accurately documenting findings, communicating critical information, differentiating conditions/symptoms and reporting to the appropriately licensed /supervisory staff and following up with triaged, appropriate clinical interventions to stabilize and treat clients.
- Conducts vital signs, medication reconciliation, check in and check out processes and/or admissions and discharges under ordered protocols, conducts testing and assessments under ordered protocols
- Analyzes and documents findings of medical and medication management, performs mental and substance use assessments, and collaborates with other team members to conduct comprehensive, consumer and to develop treatment plans
- Provides consumers and families with education on psychotropic medications, related health conditions and monitors medication compliance; supports and provides quality client care through effective assessment, individual and supportive therapies
- Provides direct client care to include but not limited to vital signs, medication injections, basic nursing and hygiene care, EKG, lab specimen collections, medication administration with follow up assessments, crises care, CPR/ First Aid as clinically indicated and/or ordered by prescriber
- Demonstrates the ability to analyze and document findings for medical and medication management; performs mental health and substance abuse assessments
- Collaborates with other team members to conduct comprehensive, consumer assessments and to develop treatment plans; assists in management of psychiatric crises and prepares written orders from the physician
- Documents clinical assessment and care findings, provides concurrent billing and medical/legal documentation meeting Diversus Health time and accuracy requirements
- Performs client care optimizing client safety and risk reduction to include but not limited to reduction in health care errors, safety planning, following safety check protocols as ordered, seeking team collaboration or supervisor input for potential risk situations, and continuously observing client
- Uses accurate knowledge of Handle with Care in safely managing crises situations with escalated clients and responds according to approved protocols for crises situations and restraint/seclusion events
