Job Title: Licensed Practical Nurse (LPN) / Registered Nurse (RN)

Location: Bluegrass Community Health Center (BCHC)

Reports to: Nurse Manager

Job Type: Full-time

FLSA Status: Non-Exempt

Position Summary:

The LPN/RN at BCHC plays a vital role in delivering high-quality patient care and supporting clinical operations in a fast-paced, team-based setting. This role requires clinical skills, a passion for serving diverse communities, and a commitment to providing care that aligns with BCHC's mission and values.

Key Responsibilities:

Clinical Duties:

  • Perform and document patient assessments, vital signs, medication administration, and immunizations.

  • Assist providers with examinations, procedures, and treatments.

  • Triage patients via phone or in-person to determine appropriate care needs.

  • Collect specimens for lab tests and ensure proper handling and documentation.

  • Administer prescribed medications and treatments in accordance with clinical protocols.

Care Coordination:

  • Collaborate with providers, care teams, and patients to support individualized care plans.

  • Provide patient education on chronic disease management, medications, preventive care, and health promotion.

  • Participate in care team huddles and interdisciplinary case management meetings.

Documentation & Compliance:

  • Accurately document all care provided in the electronic health record (EHR).

  • Follow all BCHC, state, and federal guidelines for infection control, safety, and confidentiality (including HIPAA).

  • Maintain required licensure, certifications, and continuing education.

Qualifications:

Required:

  • LPN or RN license in the state of Kentucky, in good standing.

  • CPR/BLS certification.

  • Clinical experience in primary care, family medicine, internal medicine, or a similar setting.

Preferred:

  • Experience working in a Federally Qualified Health Center (FQHC) or with underserved populations.

  • Bilingual (Spanish/English or other languages) highly preferred.

  • Familiarity with electronic health records (eClinicalWorks preferred).

  • May require occasional evening or weekend shifts depending on clinic needs.
