The team is led by a consultant team who work alongside the clinical nurse specialists. There is strong multi-disciplinary working and close links with other providers. The services is evolving and has strong educational and research focusses. Main duties of the job The post holder will assist in the delivery of high quality Hepatology services cross site throughout the Trust. Working alongside other clinical colleagues to provide expert care, treatment and support for this patient group. The post holder will contribute to advancing the body of knowledge through education, research and audit as well as developing practice. They will play a key role within the multidisciplinary team in order to ensure a seamless, co-ordinated and timely approach to care. Detailed job description and main responsibilities Clinical / direct care practice To receive referrals within the specialty from the health care team and respond to requests for expert advice and guidance To work within an agreed job plan to meet service needs which should be reviewed annually To work cross site at both Chelsea and Westminster Hospital and West Middlesex Hospital in addition to outreach clinics run by the service To conduct holistic assessment and providing supervision & guidance to colleagues in skilled history taking, physical examination, observation and ordering, conducting and interpreting relevant tests and investigations To work in collaboration with the multi professional team, contributing expert, evidence based knowledge and skills to relevant aspects of the patients care and treatment To maintain boundaries of own practice within agreed guidelines and protocols, use professional judgment in managing complex and unpredictable situations and seek guidance from other relevant experts To address patient's risk factors and health promotion needs through comprehensive assessment of individuals for risk factors and early signs of illness To build an environment of trust and rapport, enabling patent empowerment in their healthcare and acting as an advocate for the patient in treatment decisions and choices To draw upon an appropriate range of multi-agency and inter-professional resources to ensure that patients receive appropriate services, making referrals to other professions and services as appropriate To use effective communication and interpersonal skills to provide information, explanation and reassurance to patients and families To plan and conduct appropriate follow up, closure or transition of care for the patient To maintain accurate and contemporaneous records of patients assessment, care and treatment Person specification Experience in the relevant specialty Essential criteria Registered Nurse Degree /Dip HE/ Teaching and Assessing Qualification Significant endoscopy experience in pre-assessment Significant experience as a Hepatology clinical Nurse specialist Ability to Lead a procedure room and co-ordinate sessions Desirable criteria Leadership qualification Professional knowledge acquired by specialist course or equivalent experience Important The closing date given is a guide only.
