We are seeking a dediced Pient Care Manager to join our hospice team. Reporting directly to the Executive Director or Administror, you will oversee pient care activities to ensure the delivery of high-quality, compliant hospice services. This leadership role involves managing clinical operions, coordining care, and developing staff to uphold the highest standards of pient care and regulory compliance.

As a Hospice Pient Care Manager, You Will:

  • O versee and direct pient care services to ensure the delivery of high-quality hospice care
  • M anage visit scheduling, including reassigned, missed, declined, or rescheduled visitsmaking decisions based on clinical judgment and priority
  • T riage incoming phone referrals and medical concerns in a timely, compassione, and professional manner
  • L ead or facilite Interdisciplinary Group (IDG) meetings, including presenting pient updes, reviewing charts, coordining care plans, and documenting minutes
  • S erve as a mentor, trainer, and clinical support to associes, supporting development, performance, and compliance
  • E nsure all care delivery meets federal, ste, and local regulory standards, as well as company policies and procedures
  • S upport performance improvement initiives, quality assessments, and operional efficiencies within your locion
  • Compension may vary within the salary range provided based on several factors including but not limited to a candides locion, experience, educion, skills, licensure, certificions and department equity. Gentiva provides associes with a comprehensive benefits and total rewards package, of which base pay is just one piece.

    Salary Range $71,615-89,520/year

About You:

Qualificions Wh Youll Bring:

  • Current RN license in the ste of employment
  • Gradue of an accredited nursing program
  • Minimum of 3 years of direct pient care experience, including 2 years in a hospice or home-based care setting
  • Demonstred ability to apply hospice principles, industry standards, and regulory compliance (Medicare, Medicaid, JCAHO, ACHC)
  • Knowledge and compassion for terminally ill pients and their families
  • Ability to lead a diverse care team while adapting to the emotional and clinical needs of pients from various backgrounds
  • Strong interpersonal, organizional, and leadership skills
  • Competency in clinical documention and technology systems

Preferred Qualificions (Not Required):

  • Bachelor of Science in Nursing (BSN)
  • Previous leadership, team management, or supervisory experience in hospice or home health
  • Familiarity with EMR systems and telehealth plforms
  • Certificion in Hospice and Palliive Nursing (CHPN)
