Home Health Licensed Practical Nurse (LPN) - Lawrenceville, GA

Make a significant impact as a Home Health Licensed Practical Nurse (LPN), supporting patients in their homes by delivering high-quality, patient-centered care. Youll play a critical role in fostering independence and recovery while providing ongoing education and compassionate support to patients and their families. This field-based position offers a consistent weekday schedule with occasional flexibility to accommodate patient needs.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Full-time, contract
• Manage 2532 points per week (roughly 35 home visits per day)

Qualifications:

• Active and unrestricted LPN license in the state of practice
• Minimum one year of recent experience in home health, hospice, or similar community-based care setting
• Proficiency with Home Care Home Base (HCHB)
• Valid drivers license, reliable transportation, and current vehicle insurance
• Current Basic Life Support (BLS) certification
• Strong time management, clinical judgment, and communication skills
• Ability to work independently and efficiently in a field-based environment

Preferred Experience:

• Familiarity with Medicare-certified home health agencies
• Knowledge of home health compliance and documentation standards
• Excellent interpersonal skills and a patient-focused approach

Primary Responsibilities:

• Provide skilled nursing visits and implement individualized care plans
• Administer medications, deliver wound care, and manage other clinical interventions as prescribed
• Monitor and report any changes in patient condition to the RN Case Manager or physician
• Educate patients and caregivers on disease management and self-care
• Ensure meticulous documentation using HCHB and maintain compliance with all regulations
• Collaborate with the interdisciplinary healthcare team to coordinate patient care
