Nurse is a client care provider. The Nurse performs a variety of nursing functions and closely assess
clients for changes in medical or nursing status under the direction and supervision of a Medical Doctor.
This position does provide care to adults and adolescents who have sustained varied forms of head trauma,
with varying degrees of injury, who may or may not have also sustained other injuries. This position will be
responsible for supervision and documentation of the clients medical/nursing needs until discharge. Among
these duties are observing and documenting clients behavior, role modeling for the clients, interacting with
clients, providing training to residential and Tideway staff in caring for the clients, insuring client safety, and
reinforcing treatment plans and Moody Neuro/Tideway policies. This position will uphold the mission,
philosophy, and goal of the organization while maintaining strict confidentiality of personnel and client data
and information in the performance of assigned duties. This position will act as a liaison between the
Medical Director, Director of CQI, Clinical Departments, and the Residential department.
The Tideway Case Manager will be responsible for providing all case management services under the
supervision of the Director of Tideway. The Tideway Case Manager shall attend all staff and quarterly
meetings to gather information, provide input to direct care staff, and facilitate the team process to
help clients maximize their rehabilitation potential. The Tideway Case Manager shall facilitate all family
conferences to assist the family/referral source to understand the client's progress from a holistic
perspective. At all times, the Tideway Case Manager will promote the client's participation in the program
by performing advocacy services for the individual. The Tideway Case Manager shall be responsible for
scheduling in order to coordinate the activities, appointments and recommended therapies of the individual's
treatment program and regularly evaluate the appropriateness of the prescribed treatment program in
relation to progress towards stated goals. The Tideway Case Manager will be responsible for regular
contact with the family/guardian of each individual served and contacting the funding source on at least a
quarterly basis. The Tideway Case Manager shall facilitate optimum rehabilitative progress of
trainees/clients served at all times. The Tideway Case Manager will be required to demonstrate flexibility in
working within the interdisciplinary structure and will uphold the mission, philosophy and goals of the
organization while maintaining confidentiality of personnel and client data, and other information in the
performance of assigned duties.
ESSENTIAL FUNCTIONS
Major Duties and Responsibilities
Nursing:
Specified nursing services as needed by Moody Neuro/Tideway clients.
Assess, monitor, intervene, document, and report clients social and interpersonal behavior and functional ability to perform as an independent individual in the community setting.
Carry out therapeutic activities as designated by the Medical Director in conjunction with the Director of CQI.
Follow behavior modification program recommendations and submit documentation to the Case Manager as appropriate.
Assist in emergency situations including the implementation of fire alarm and drill procedures, and the provision of first aid treatment in the case of illness or injury, in respect to triage procedures.
Monitor client self-administration of medications and documentation of ability to perform this function.
If accompanying clients on field trips, etc., will act as medical supervisor as needed.
Provide documentation of observation of client responses on outings as needed.
Assist with specified therapies at the request of the Medical Director or Director of CQI.
Serve as a role model at all times for the clients and residential staff.
Attend in-services, workshops, staff meetings, and seminars, as directed.
Maintain confidentiality of both client and staff issues.
Provide safe and secure environment for clients and staff.
Discharge duties in accordance with Moody Neuro/Tideway organizational policies, CARF, and other accrediting/licensing bodies.
Assist in the training of Residential and Tideway staff as directed by the Medical Director and Director of CQI.
Perform INR/PT testing.
Other duties as assigned by the Director of CQI or Medical Director.
Organizational Duties:
Maintain medical documentation, including, but not limited to, medication monitoring sheets, incident reports, and medical consults.
Act as a therapeutic liaison between the Medical Director, Director of CQI, and the Residential/Tideway staff.
Maintain security procedures according to Moody Neuro/Tideway policies.
Maintain appropriate records of work schedule on timecard.
Case Management:
Conducts pre-admission screenings with support of Business Development.
Reviews in a timely manner the client's pre-admission documentation and information gained from the pre-admission screening and shares this information with relevant staff.
Conducts family interview during intake process.
Assures that reviews of the clients progress, case management notes, and any other relevant documentation are forwarded to the program record.
Attends all staff and quarterly meetings concerning clients being served on his/her caseload.
Performs advocacy services for the client during implementation of the client's treatment plan.
Discusses plans, goals, and the status of the individual being served, with the client and significant others to ensure client involvement and family education on an ongoing basis.
Provides a copy of the treatment plan to the family/guardian and funding source after proper releases are obtained.
Assists with scheduling medical appointments when the client is unable to schedule them him/herself and the family is not in the Houston/Galveston area.
Explains the rules and regulations of the family and rights of the individual being served relative to conflict resolution.
Assists the individual and family regarding travel arrangements for weekends, holidays, and emergencies.
Assists with the monitoring of any medication problems in coordination with physician/facility nurse.
Coordinates and assists with development of discharge plans for the individual.
Provides appropriate referral information to discharge community or alternate placement.
Works on-call as required and available after hours as needed.
QualificationsRequired Knowledge, Skills, and Abilities:

Current Texas state license as a RN.
Knowledge of TBI and related syndromes.
Ability to work well with other people in a clinical setting.

Knowledge of the multiple and complex needs of the brain injured population.
Knowledge of, and ability to document according to facility standards.
Excellent communication skills.
Ability to work as a team member and facilitate the efforts of the treatment team.

Knowledge of licensing and accreditation standards (i.e., CARF, TRC, Medicare/Medicaid).
LIFTING REQUIREMENTS - individuals in clinical care positions may be required to assist with transfers up to 150
pounds and should have the ability to lift to 100 pounds without assistance.
EDUCATION
Graduate of an accredited school of nursing. Hold a current license to practice as a Registered Nurse in the
state of Texas.
Required CPR certification.
Required Texas driver's license.

Prefer 2 years post-graduate experience in nursing, preferably in the fields of rehabilitation, patients with
cognitive deficits, or mental health issues.
