Job Summary

Summary
Responsible for coordinating and facilitating the safe and efficient transfer of patients within and between healthcare facilities. This position involves collaboration with healthcare teams, communication with external facilities, and ensuring a smooth transition of care for patients.

Essential Functions
-Coordinate the transfer of patients within the hospital or between healthcare facilities.
-Serve as the primary point of contact for communication with sending and receiving facilities.
-Conduct patient assessments to determine the clinical status and medical needs before transfer.
-Ensure that necessary medical documentation and patient records accompany the transfer.
-Complete and maintain accurate documentation related to patient transfers, including medical records, transfer summaries, and communication logs.
-Coordinate the allocation of necessary resources for patient transfers, including appropriate medical personnel, equipment, and transportation.
-Provide support and information to patients and their families regarding the transfer process.
-Address concerns and ensure that patients and families are informed and comfortable with the transfer plan.

Education
Bachelor's Degree Nursing required

Licenses and Credentials
Registered Nurse [RN - State License] - Generic - HR Only required Basic Life Support [BLS Certification] - Data Conversion - Various Issuers required

Experience
Clinical experience in acute care or critical care settings 2-3 years required and Patient transfer experience 1-2 years ideal

- Knowledge of regulatory requirements and standards related to patient transfers.
