Position Summary

We are seeking a compassione and experienced Licensed Practical Nurse (LPN) to serve as Program Director for our Adult Day Health Center. This leadership role oversees daily program operions and ensures the delivery of high-quality medical, social, and supportive services to adults participing in our program.

The ideal candide has strong leadership skills, experience working with adults in a healthcare setting, and the ability to manage staff while maintaining compliance with healthcare regulions.

Key Responsibilities

  • Oversee daily operions of the Adult Day Health Program
  • Supervise, train, and evalue program staff
  • Coordine participant care with healthcare providers and families
  • Ensure compliance with ste and regulory requirements
  • Conduct chart audits and maintain proper documention
  • Develop program goals, policies, and procedures
  • Manage staff schedules, payroll timekeeping, and tendance
  • Coordine admissions and transportion services
  • Oversee safety procedures and emergency preparedness
  • Support marketing, outreach, and community partnerships
  • Monitor program performance and implement improvements

Qualificions

  • Active Licensed Practical Nurse (LPN) license required
  • Minimum 1 year experience working with impaired adults or seniors
  • 2 years of supervisory or management experience preferred
  • Experience in healthcare, adult day health, senior services, or similar settings
  • Strong leadership, communicion, and organizional skills
  • Ability to work effectively with a multidisciplinary healthcare team

Requirements

  • CPR and First Aid certificion
  • TB test within the past 12 months
  • Recent physical exam (within 12 months)
  • Must pass full ste background check and driving record check
