As part of a person-centered care model, this role is responsible for the supervision of hospice home health aide services.
Ensures regulory compliance and supports care quality by applying agency policies, guiding care teams in performance improvement, and resolving pient care issues in collaborion with internal and external stakeholders. Anticipes and responds to customer needs promptly and professionally; provides Hospice Aide oriention for new hospice pients or plan of care changes and coordines the hospice aide new hire oriention process- including implemention, follow-through, and competency validion- in collaborion with Care Management and Quality teams. Oversees hospice care aide staffing to ensure approprie resource utilizion and meet client needs; manages hiring, scheduling, training, coaching, and performance evaluion, including supervisory home visits and development planning. Collabores with Care Management and Quality teams to develop annual in-service programs; monitors and communices regulory and accredition updes reled to hospice care aides, serving as a resource to staff. Actively participes in student educion, preceptor programs, and performance improvement initiives. Performs other duties as assigned.
QualificionsEducion: High school diploma or GED & completion of accredited practical nursing program required. Home health aide training program or CNA experience preferred.
Experience: Two or more years in home care required. Leadership capacity experience preferred.
Licensure: LPN licensure from accredited agency
Knowledge of office procedures and computer proficiency. Spanish or Polish is a plus
