Home Health Licensed Practical Nurse in Jonesboro, GA

Deliver vital care and support to individuals in the comfort of their homes as a skilled Home Health Licensed Practical Nurse (LPN). Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Flexibility may be needed based on patient needs

Key Qualifications:

• Active, unrestricted LPN license
• Minimum 1 year of home health, hospice, or community-based care experience
• Strong proficiency with Home Care Home Base (HCHB) charting system
• Valid drivers license and reliable, insured vehicle
• Current BLS certification
• Clinical acumen and strong communication skills
• Ability to independently manage a dynamic caseload in a field-based setting

Preferred Background:

• Experience with Medicare-certified home health agencies
• In-depth understanding of home health documentation and compliance standards
• Effective interpersonal skills and a patient-centered approach

Key Responsibilities:

• Conduct skilled nursing visits, providing individualized care and patient assessments
• Administer medications, wound care, and clinical interventions as prescribed
• Observe and report changes in patient condition to the RN Case Manager or provider
• Educate patients and caregivers on disease management and safe self-care
• Accurately document care and communication in Home Care Home Base (HCHB)
• Collaborate with the interdisciplinary healthcare team to coordinate care and maintain compliance
• Adhere to infection control policies and participate in ongoing training and case conferences
