Home Health Registered Nurse (RN) Lenexa, Kansas

Experienced and compassionate registered nurses with a background in home health are invited to apply for a vital field-based opportunity. This role focuses on providing comprehensive care to patients in their homes, emphasizing continuity, patient-centeredness, and clinical excellence. As a Home Health RN, you'll work independently, fostering positive outcomes by coordinating care, educating patients and families, and maintaining open communication with an interdisciplinary healthcare team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload expectation: 2532 points per week (typically 35 patient visits per day)
• Daily travel required within assigned territory (mileage reimbursement provided)

Key Responsibilities:

• Complete thorough OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, promptly addressing any changes and reporting to physicians and team members.
• Educate patients and families on treatment plans, medications, disease processes, and prevention strategies.
• Ensure accurate, timely charting within the Home Care Home Base (HCHB) system.
• Collaborate closely with physical and occupational therapists, social workers, and aides for coordinated care.
• Attend case conferences, participate in staff meetings, and engage in continuing education.
• Adhere to all applicable regulatory and professional standards.

Qualifications:

• Active RN license in good standing
• Minimum 1 year of recent home health field experience
• Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS
• Reliable transportation, valid driver's license, and auto insurance
• Strong clinical assessment skills, organizational ability, and communication proficiency
• BSN and familiarity with Medicare/home health regulations are preferred
