Home Health Registered Nurse in Overland Park, KS

A highly skilled, compassionate RN is needed for a 13-week contract providing home health nursing services. This field-based position focuses on delivering individualized care and empowering patients to thrive in their home environment. As a Home Health Registered Nurse, you will assess patients, implement evidence-based care plans, and foster collaboration across interdisciplinary teams.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform OASIS-driven assessments and create individualized, measurable care plans.
• Deliver skilled nursing interventions: wound care, IV therapy, medication administration, chronic disease management.
• Monitor patient progress, identify any changes in condition, and update the care team and physicians as needed.
• Educate patients and families regarding disease management, medications, and preventive health practices.
• Accurately document all visits and care provided using Home Care Home Base (HCHB).
• Coordinate with other healthcare professionals (physical therapists, occupational therapists, social workers, and home health aides) to deliver comprehensive care.
• Attend case conferences, contribute to staff meetings, and participate in continuing education.
• Maintain compliance with regulatory standards and organizational protocols.

Required Qualifications:

• Active, unencumbered RN license in the state of practice
• At least 1 year of recent home health experience (field-based)
• Proficiency using Home Care Home Base (HCHB); OASIS experience required
• Strong assessment, clinical, and problem-solving abilities
• Reliable vehicle, valid drivers license, and proof of auto insurance
• Excellent communication and organization; ability to work independently

Preferred:

• Bachelor of Science in Nursing (BSN)
• Experience managing caseloads of 2532 points weekly
• Familiarity with Medicare requirements and home health compliance
