Home Health Registered Nurse (RN) Olathe, Kansas

An exciting contract opportunity is available for an experienced and compassionate Registered Nurse with a background in home health care. In this field-based role, youll provide high-quality, patient-centered nursing services to clients in their homes, ensuring continuity of care and positive patient outcomes. Schedule and Setting:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Field-based, requiring daily travel within an assigned territory (mileage reimbursement provided)
• Electronic documentation using Home Care Home Base (HCHB); OASIS assessment tool

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop personalized care plans
• Deliver skilled nursing care including wound management, medication administration, IV therapy, and chronic disease oversight
• Regularly evaluate patient progress and update the care team and physicians on changes in condition
• Educate patients and families regarding diagnoses, treatments, medications, and preventive strategies
• Complete timely and accurate documentation in HCHB
• Work alongside physical therapists, occupational therapists, social workers, and home health aides to coordinate care
• Participate in case conferences, continuing education, and staff meetings
• Ensure strict compliance with agency and regulatory standards

Qualifications:

• Active RN license (in good standing)
• At least 1 year of recent home health experience (field preferred)
• Proficient with HCHB and OASIS
• Strong clinical assessment, communication, and organizational skills
• Reliable transportation, valid drivers license, and auto insurance
• BSN and knowledge of Medicare/home health standards preferred
