Licensed Practical Nurse Home Health Bedford, New Hampshire

Deliver compassionate and high-quality patient care as a Home Health Licensed Practical Nurse (LPN). This field-based role is ideal for professionals skilled in both clinical excellence and strong patient communication, supporting individuals on their journey to greater independence and recovery within the comfort of their own homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)

Essential Qualifications:

• Active and unrestricted LPN license in the state of practice
• At least one year of home health, hospice, or related community-based care experience
• Proficient in Home Care Home Base (HCHB) charting system
• Valid drivers license and reliable transportation with current insurance
• Current BLS (Basic Life Support) certification
• Excellent time management, clinical judgment, and communication skills
• Ability to independently manage a dynamic and mobile caseload

Preferred Experience:

• Knowledge of Medicare-certified home health agencies
• Solid grasp of home health compliance and documentation standards
• Outstanding interpersonal skills and a deeply patient-centered approach

Key Responsibilities:

• Conduct comprehensive in-home nursing assessments and develop individualized care plans
• Administer medications, wound care, IV therapy, and other physician-ordered interventions
• Observe, document, and promptly report changes in patient condition
• Deliver education to patients and caregivers on medication management, disease processes, and self-care
• Maintain timely and accurate documentation using HCHB
• Collaborate with an interdisciplinary team, participate in case conferences, and uphold infection control standards
