The Hospice RN Case Manager is responsible for independent management of the Hospice patient population requiring use of advanced assessment, teaching and decision-making skills. The Hospice RN Case Manager cares for a caseload of hospice patients by evaluating the patient for appropriateness of hospice care and developing the plan of care in collaboration with the provider, caregivers and Hospice IDG members. The Hospice RN Case Manager provides follow up by evaluating the effectiveness of the plan of care and monitoring the patient and caregivers response to the plan. The Hospice RN Case Manger provides patient care based on providers orders, departmental policies and procedures, regulatory guidelines and best practice. The Hospice RN Case Manager provides care in collaboration with the provider, patient, caregivers, and Hospice IDG members to meet the goals of care with the consideration of the patients limitations and personal preferences in accordance with hospice philosophy. The Hospice RN Case Manager documents in the patients medical record in a thorough and timely manner according to agency requirements. The Hospice RN Case Manager attends and actively participates in IDG and team meetings. The Hospice RN Case Manager performs on-call responsibilities and Holiday staffing. Other Items of Note:
Graduate from an accredited Diploma of Nursing Program or Associates Degree in Nursing. Preferred Education:
Bachelors Degree in Nursing
Required Skills:
Nursing skills as defined by generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional and efficient verbal and written communication skills. Good interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (copy machine, fax machine, etc.) Ability to be open, sensitive, flexible and ecumenical. Must have demonstrated experience in working with patients and caregivers dealing with life-threatening illness and end of life. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.
Preferred Skills:
Home Care Home Base experience; Experience with smart phones and smart devices Required Licensure/Registration/Certification:
Current Texas state license in appropriate discipline; Current American Heart Association CPR; Current Texas state drivers license; Current auto insurance. Must have reliable transportation.
Preferred Licensure/Registration/Certification:
Hospice and Palliative Care certified; Wound Care Certified; Advanced education or certification in area of specialty.
Required Experience:
Two years experience in healthcare setting in respective discipline
Preferred Experience:
Medical-Surgical experience; Experience in Case Management; Supervisory position in healthcare setting; Home Care or Hospice experience
