Hospice of the Piedmont is seeking an experienced and dedicated Registered Nurse to lead the daily operations of our Hospice House. In this role, you will combine clinical expertise with leadership to ensure exceptional, compassionate care for patients and families facing serious illness and end-of-life transitions. Youll play a critical role in maintaining quality, regulatory excellence, and a supportive care environmentwhile guiding a team committed to dignity, comfort, and respect at every stage of the journey.
Responsibilities:
Clinical Care
- Provides direct patient nursing care to HOP House residents
- Develops the Plan of Care (POC) in conjunction with the physicians orders and in coordination with other interdisciplinary team members assessment of patient/family needs, condition, and environment
- Regularly re-evaluates patient needs, consulting with patients physician, reporting changes which might necessitate a change in the POC and communicates this with HOP employees and contractors involved in the patients care
- Responsible for obtaining medications on admission and any new orders while on duty. Regularly leads IDT (Interdisciplinary Team) meetings to coordinate the POC with approval of the Medical Director and communicates any changes to the appropriate House staff
- Performs narcotic count daily and adheres to policy/procedure for safekeeping of all medications and the destruction of medications with proper documentation.
- Updates RMA and CNA on duty with changes and write it on report sheet for next shift, update POC as needed.
- Utilizes the electronic medical system for running reports and documentation and is a resource for staff when indicated
Supervisory
- Has oversight of the delivery of all skilled services provided to patients on the team on which (s)he serves.
- Communicates frequently with the Director of Homecare Services seeking direction on issues that affect services, quality of care, admission/discharge decisions, and overall operations of the House.
- Provides guidance and direction to staff reporting to him/her.
- Coordinates with Admissions department and for admission to the House.
- Assists with training/orientation of departmental staff.
- Supervises documentation for other staff members to ensure it is appropriate and complete.
- Reviews and adjusts staff work duties as required.
- Completes annual performance reviews of staff who routinely are under your supervision.
Administrative / General
- Maintains health, fire, and safety measure according to policy and procedure.
- Participates in Quality Improvement teams, audits or projects as assigned.
- Follows HOP personnel policies and procedures (i.e., recruitment and hiring process, completion of incident reports, etc.).
- Responsible for regulatory compliance and survey readiness in partnership with QAPI department.
- Responsible for ensuring food and supplies are ordered in a timely manner.
- Adheres to the practice of confidentiality regarding patients, families, staff, and the agency.
- Reports any care issues or activities that create a variance (Incident) report timely to the Director of Hospice Homecare and QAPI, as appropriate.

Qualifications :
- Licensed as a Registered Nurse in the Commonwealth of Virginia
- Five years relevant nursing experience required
- Nursing home/assisted living facility experience preferred
- Drivers license, personal automobile, and current automobile liability insurance

- Due to the nature of services provided, it may be necessary at times for employees to work beyond scheduled work hours to ensure adequate patient care
- Requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye-hand coordination.
- The employee must occasionally lift and/or move 75-100 pounds and push up to 200 pounds in a wheelchair.
- Physical and emotional health necessary for the accomplishment of position duties.
