Licensed Practical Nurse Home Health Little Rock, Arkansas

Deliver meaningful care and make a difference every day as a Home Health Licensed Practical Nurse. In this field-based role, you will provide direct nursing services to individuals in their homes, supporting their independence through compassionate, patient-focused care. Your contributions will empower patients in their recovery and educate both patients and caregivers on essential health topicsall while collaborating with a dedicated interdisciplinary team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional flexibility may be required to meet patient needs

Key Responsibilities:

• Conduct skilled nursing visits in patient homes, assessing, planning, and implementing individualized care plans
• Deliver clinical interventions such as medication administration, wound care, and IV therapy as ordered
• Monitor patient conditions closely, reporting changes to the RN Case Manager or physician promptly
• Educate patients and family members about disease processes, medication regimens, and self-care
• Document care and communications accurately via Home Care Home Base (HCHB)
• Work collaboratively with the interdisciplinary team to ensure coordinated, compliant care
• Uphold infection control and safety standards
• Engage in case conferences, ongoing quality improvement initiatives, and required training

Qualifications:

• Active, unrestricted LPN license in the state of practice
• Minimum of 1 year of experience in home health, hospice, or a similar setting
• Proficiency in Home Care Home Base (HCHB) is required
• Valid drivers license, reliable vehicle, and current insurance
• Current BLS certification
• Excellent clinical judgment, time management, and communication skills
• Comfort working independently with a dynamic patient caseload

Preferred:

• Background with Medicare-certified agencies and in-depth understanding of home health compliance standards
