Home Health Licensed Practical Nurse (LPN) - Tucson, Arizona

This contract opportunity as a Home Health Licensed Practical Nurse (LPN) offers the chance to work directly in patients homes, fostering independence, education, and recovery while collaborating closely with both families and the wider healthcare team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional flexibility for patient needs

Key Qualifications:

• Active, unrestricted LPN license in the state
• At least 1 year of experience in home health, hospice, or related community-based care
• Proficiency with Home Care Home Base (HCHB) for documentation and compliance
• Current BLS certification
• Valid drivers license and reliable vehicle with current insurance
• Strong clinical judgment, time management, and excellent communication skills
• Ability to work independently in a field-based environment

Preferred Experience:

• Background with Medicare-certified home health agencies
• Thorough understanding of home health compliance and documentation
• Demonstrated patient-centered communication and interpersonal skills

Primary Responsibilities:

• Conduct skilled home visits, assessing and implementing individualized care plans
• Provide clinical interventions such as medication administration, wound care, or IV therapy
• Promptly report changes in patient status to appropriate team members
• Educate patients and caregivers on care techniques and health management
• Document assessments and interventions using HCHB in line with compliance requirements
• Coordinate closely with the interdisciplinary team to ensure high-quality care
• Maintain infection prevention and enforce patient safety practices
• Participate in case conferences, training, and quality improvement efforts
